APPOINTMENT CONTROL
We strive to be on time for your appointment; we do not double appoint; if you cancel your appointment, this prevents others from being seen who need our services. Therefore, a 48-hour notice of cancellation is requested to avoid a cancellation fee. The use of our answering machine to cancel appointments is not accepted; please call our office during our regular business hours. We recognize that emergencies do occur and will be understood.
FEES FOR SERVICES
The patient is responsible for the entire fee for the services rendered; we accept insurance as a courtesy to our patients who have dental benefits. However, the patient and the insurance company are the parties who have an agreement; our sole concern is that the patient receives the best dental care.
INSURANCE
Our staff is trained and experienced in applying for all the benefits available to our patients; the patient is responsible to provide our staff with all insurance benefit information or any changes/updates that may occur.
TREATMENT CHOICES AND FEES FOR SERVICE
All our patients receive a written statement explaining the service to be rendered and the fees associated with this treatment before the treatment is started. This form is to be signed and dated so that our staff is dilligent in keeping this policy.
MEDICAL HISTORY AND UPDATES
We ask that any changes in medical condition, new drug therapy, or allergies be stated to the office personnel prior to any dental procedure.
OVERDUE ACCOUNTS
We realize that financial emergencies do occur. Our staff can assist you in the best way to complete your financial commitment.
